Prepare to be enchanted by a truly sensational Italian pasta dish: **Creamy Pistachio Pasta with Crispy Guanciale**. This culinary masterpiece features tender tagliatelle coated in a rich, velvety pistachio sauce, crafted with fresh garlic, aromatic basil, and grated Pecorino Romano cheese. Each forkful is elevated by the irresistible crunch and savory depth of crispy guanciale, making for an unforgettable meal that is both comforting and surprisingly elegant. Best of all, this beautiful dish can be on your table in under 30 minutes, proving that gourmet flavors don’t always require hours in the kitchen.

For those who, like us, possess an unyielding passion for pistachios, this recipe is more than just a dish – it’s a revelation. From the sweet indulgence of pistachio ice cream and decadent pistachio cake to the ethereal smoothness of Italian pistachio cream and even savory delights like pistachio-crusted chicken, the versatility of this exquisite nut knows no bounds. If you’re captivated by the unique, subtly sweet, and earthy flavor of pistachios, then you are guaranteed to fall head over heels for this pasta creation. It brings together all the best elements of Italian cuisine with a delightful, nutty twist.
The secret to this pasta’s incredible taste lies in its vibrant, homemade **pistachio pesto**. Unlike many store-bought varieties, our pesto is a fresh blend of premium pistachios, fragrant basil, a hint of garlic, rich Pecorino Romano, and high-quality olive oil. This flavorful base is then transformed into a gloriously silky-smooth sauce with the addition of luxurious heavy cream, creating a coating for the pasta that is both deeply satisfying and addictively delicious. The harmonious balance of flavors makes this dish truly special.
But what truly elevates this creamy pistachio pasta from simply delicious to absolutely unforgettable is the generous topping of **crispy guanciale**. This Italian cured pork cheek, with its rich, salty, and melt-in-your-mouth texture when perfectly rendered, adds an essential layer of flavor and a delightful textural contrast. Its savory crispiness cuts through the creaminess of the sauce beautifully, providing a delightful counterpoint that is, in our humble opinion, an absolute must. Pair this incredible meal with a fine glass of Italian vino, and you have the makings of a truly spectacular dining experience right in your home.
Ingredients for Your Creamy Pistachio Pasta

Ingredient Notes and Expert Tips for Substitutions
- Pistachios: For the most authentic and vibrant flavor in your pistachio pesto, always opt for **unsalted and unroasted pistachios**. Roasting changes their delicate flavor profile, and salted nuts would require careful adjustment of seasoning, which can be tricky. Look for shelled pistachios to save time.
- Guanciale: This is the star cured pork product, specifically Italian cured pork cheek. It boasts a rich, complex flavor and a beautiful fat that renders down to incredible crispiness. If guanciale is challenging to find in your local Italian deli or specialty store, **pancetta** is the best substitute. While pancetta is cured pork belly, it offers a similar savory element and can be cooked to a delightful crispness. Avoid using regular bacon as it has a smoky flavor that will alter the dish’s profile.
- Cheese: We highly recommend using a combination of **Pecorino Romano** and **Parmigiano Reggiano** for a balanced depth of flavor. Pecorino Romano, made from sheep’s milk, provides a sharp, salty kick, while Parmigiano Reggiano offers a nuttier, more nuanced taste. Feel free to use both, or if you must choose just one, Parmigiano Reggiano is often more widely available and provides a good base. Ensure you’re grating fresh cheese for the best results; pre-grated cheeses often contain anti-caking agents that can affect sauce texture.
- Garlic: You might notice the recipe calls for just **half a clove of garlic**. This is intentional! We aim for a subtle hint of garlic to complement the pistachios and basil, not to overpower them with a strong, raw garlic flavor. Adjust to your personal preference, but start small.
- Pasta: While we adore **Tagliatelle** for its broad, flat ribbons that perfectly cling to the creamy sauce, this pistachio pasta recipe is highly versatile. Any long, flat pasta like fettuccine or linguine would work beautifully. Shorter shapes such as penne or rigatoni can also be used if preferred, especially if you want to capture more sauce in each piece. Always cook your pasta al dente for optimal texture.
- Heavy Cream: Full-fat heavy cream is crucial for achieving the desired rich and luxurious consistency of the sauce. Do not substitute with lighter creams or milk, as they will not thicken properly and will result in a less decadent dish.
- Olive Oil: Use a good quality extra virgin olive oil for the pesto. Its fruity notes will enhance the overall flavor without being too assertive.
- Salt and Pepper: Always season to taste. Remember that guanciale and Pecorino Romano are salty, so add salt incrementally. Freshly ground black pepper adds a lovely subtle spice.
Crafting Your Creamy Pistachio Pasta: A Step-by-Step Guide
Creating this gourmet-level pistachio pasta is simpler than you might imagine. Follow these detailed steps for a perfect meal every time. The full recipe card with precise ingredient amounts can be found below.
1. Prepare the Pistachio Pesto
The foundation of our incredible sauce is a vibrant, homemade pistachio pesto. To make it, combine all the pesto ingredients (unsalted, unroasted pistachios, fresh basil, ½ clove garlic, Pecorino Romano, and extra virgin olive oil) in a bowl. Use an immersion blender to blitz the mixture until it forms a smooth, creamy paste. Don’t worry if a few larger pistachio chunks remain; they add a wonderful texture. Season with a pinch of salt and freshly ground pepper to taste.

2. Render the Crispy Guanciale
To achieve perfectly crispy guanciale, begin by trimming any tough skin or grainy seasoning from the pork cheek, then cut it into even strips or small cubes. Place the guanciale in a cold pan, then turn the heat to medium-low. This gradual heating allows the fat to render slowly and evenly, preventing it from burning and becoming bitter. Fry until the guanciale pieces are beautifully golden brown and crisp. Remove the crispy guanciale with a slotted spoon and transfer it to a plate lined with paper towels to drain any excess fat. Carefully pour the remaining rendered fat from the pan into a separate ramekin; you can discard this once it solidifies, or save it for future cooking to add incredible flavor to other dishes.

3. Cook the Pasta
While the guanciale renders and you prepare the sauce, bring a large pot of water to a rolling boil. Remember to generously salt the pasta water; it’s your first chance to season the pasta itself, enhancing its flavor. Add your tagliatelle (or chosen pasta shape) to the boiling water and cook according to package directions until it is al dente – tender with a slight bite. Tagliatelle typically cooks in about 5 minutes, which is the perfect amount of time to simultaneously prepare your creamy sauce.
4. Create the Creamy Pistachio Sauce
Do not wipe the pan clean after frying the guanciale; the residual flavor is essential! Add the heavy cream to the same pan and bring it to a gentle simmer over medium-low heat. Once the cream begins to thicken slightly, reduce the heat to very low or turn it off. Stir in the finely grated Parmigiano Reggiano and a pinch of salt and pepper until the cheese has completely melted and is fully incorporated. Immediately after, add your prepared pistachio pesto to the creamy mixture. Mix thoroughly until the pesto is evenly distributed, creating a vibrant, uniform green sauce.

5. Adjust Sauce Consistency
At this stage, your sauce might be quite thick. To achieve the perfect silky, rich consistency, gradually add small splashes of the reserved pasta water. The starchy pasta water helps to emulsify the sauce, making it incredibly smooth and allowing it to cling beautifully to the tagliatelle. Add just a tablespoon at a time, stirring well after each addition, until the sauce reaches your desired creamy yet flowing texture.
6. Combine and Serve
Once your pasta is perfectly al dente, use tongs to transfer the cooked tagliatelle directly into the pan with the creamy pistachio sauce. Toss the pasta vigorously to ensure every strand is thoroughly coated in the luxurious sauce. The pasta will continue to absorb the flavors as you mix. Serve immediately in warm bowls, generously topping each serving with the reserved crispy guanciale. A final sprinkle of fresh Pecorino Romano or a few extra pistachio crumbs can add an extra touch of flair.

Expert Tips for the Perfect Pistachio Pasta
- Prep Ahead for Ease: To make this dish even quicker on a weeknight, prepare your pistachio pesto in advance. It can be made a day or two ahead of time and stored in an airtight container in the refrigerator, topped with a thin layer of olive oil to prevent oxidation. This small step significantly reduces the active cooking time.
- Mastering Guanciale Frying: The key to perfectly crispy, flavorful guanciale (or pancetta) is to start with a cold pan and use a medium-low heat. Adding guanciale to a hot pan will cause it to seize and burn quickly, resulting in a bitter taste and less rendered fat. The slow rendering process ensures maximum crispiness and flavor extraction.
- Achieving Ideal Sauce Consistency: Cream-based sauces tend to thicken quickly. If your pistachio sauce becomes too thick, especially after adding the cheese, gradually add small amounts of pasta cooking water. The starch in the pasta water helps to emulsify the sauce, making it wonderfully smooth and glossy without thinning out the flavor. Aim for a thick yet silky-smooth consistency that beautifully coats the pasta.
- Storage and Reheating Leftovers: Like most cream-based pasta sauces, this dish is best enjoyed immediately for optimal texture and flavor. The sauce will thicken considerably as it cools. If you have leftovers, store them in an airtight container in the refrigerator for up to 2-3 days. When reheating, gently warm the pasta on the stovetop over low heat, adding a splash of water or a little extra cream to loosen the sauce back to its original consistency.
- Quality Ingredients Matter: Since this recipe uses a few key ingredients, the quality of each truly shines through. Invest in good quality unsalted pistachios, fresh basil, authentic Pecorino Romano, and a flavorful guanciale for the best possible outcome.

Serving Suggestions and Wine Pairings
This creamy pistachio pasta is a complete meal on its own, but it can be beautifully complemented with a simple side dish. A crisp green salad with a light vinaigrette would offer a refreshing contrast to the richness of the pasta. For an extra touch of Italian authenticity, serve with warm crusty bread to sop up every last drop of the delectable sauce. When it comes to wine, a dry white wine with good acidity and mineral notes would pair wonderfully. Consider a Vermentino, a Pinot Grigio, or even a light Chardonnay to balance the creamy texture and nutty flavors of the dish.
Frequently Asked Questions About Pistachio Pasta
- Can I make this pistachio pasta vegetarian?
- Absolutely! To make this dish vegetarian, simply omit the guanciale. For added depth, you can sauté some mushrooms or sun-dried tomatoes in olive oil before making the sauce. You can also use a vegetarian hard cheese instead of Pecorino Romano if needed.
- What if I don’t have an immersion blender for the pesto?
- A regular food processor will work perfectly for making the pistachio pesto. Just ensure you process it until it reaches a smooth consistency, scraping down the sides as needed.
- Can I use roasted pistachios?
- While we recommend unsalted, unroasted pistachios for the best flavor, if roasted pistachios are all you have, make sure they are unsalted. The roasted flavor will be more intense and slightly different from the delicate taste of raw pistachios.
- Is this dish spicy?
- No, this dish is not inherently spicy. The garlic is used sparingly for a subtle aromatic note. If you prefer a touch of heat, you can add a pinch of red pepper flakes to the guanciale as it fries or into the sauce.

Ingredients
For the Pistachio Pesto
- ½ cup (70g) unsalted unroasted pistachios
- 1 cup (10g) fresh basil
- 1/2 clove garlic
- ½ cup (30g) Pecorino Romano, finely grated
- 4-5 tablespoons (50ml) extra virgin olive oil
- salt and pepper to taste
Rest of the Ingredients
- 14 oz (400g) pasta, we used tagliatelle
- 3.5 oz (100g) Guanciale, can also use pancetta
- 1 cup (225ml) heavy cream
- 1/3 cup (25g) Parmigiano Reggiano, finely grated
Instructions
-
Bring a large pot of water to a boil and salt it well.
-
Meanwhile, put all ingredients for the pesto in a bowl and blitz to a smooth paste using an immersion blender (or food processor). Season with a pinch of salt and pepper and set aside.
-
Trim the tough skin and grainy seasoning from the Guanciale then cut it into strips. Using a large pan, add the guanciale to a cold pan then turn to a medium-low heat. Fry until golden and crispy. Do not let it burn or it’ll turn bitter.
-
Remove the crispy Guanciale to a plate and pour any remaining fat into a separate ramekin to discard later.
-
Add your pasta to the boiling water and cook until al dente according to package instructions (tagliatelle usually takes around 5 minutes).
-
Meanwhile, add the heavy cream to the same pan you cooked the guanciale in (do not wipe clean). Bring to a simmer. Turn the heat right down, add the Parmigiano Reggiano and a pinch of salt and pepper. Stir until melted then turn off the heat.
-
Add the pistachio pesto to the cream and stir until fully combined. Add a little splash of pasta water to loosen the sauce to a silky consistency if needed.
-
When your pasta is ready, add it directly to the pan with the sauce and toss until completely coated. Serve immediately in bowls topped with the crispy guanciale.
Notes
- Prep ahead: Make your pistachio pesto first and set it aside. You can even do this the day before.
- Frying guanciale: Make sure to do this on a medium to low heat. Add your guanciale to a cold pan then turn on the heat as it burns very easily and will turn bitter.
- Sauce consistency: Adjust the consistency using pasta water to loosen it. It should be a thick but silky-smooth sauce.
- Storage and leftovers: Like most cream-based sauces, this will thicken quite a bit as it cools. It’s best eaten immediately although can be stored in the fridge and reheated (loosen the sauce with a little water).
